TREATMENT OF A MINOR

California law authorizes the parent(s) or guardian of a minor child (i.e. anyone under the age of eighteen) to give consent for medical treatment. Minors should be accompanied by a parent or guardian at their initial appointment.

A parent or guardian will be asked to sign to authorize treatment. Please review our Authorization for Treatment and Assignment of Benefits form in the Patient Forms section of our website.
